Does anyone know where I can find this quick-release plate for my tripod?

I got a Velbon DV-48 tripod with a Vel-flo 8 PH-258 fluid head, but it doesn't have a quick release plate. Does anyone know what model number the release plate is or where I could find one? Thanks a bunch.
